カメのかかりやすい病気「肺炎」

今回は、カメの肺炎についてお話しします。
カメの肺炎の病態
カメの下部呼吸器感染症、いわゆる肺炎の原因としては、細菌、ウイルス、真菌、寄生虫が挙げられています。臨床例の多くは不適切な湿度や温度管理など飼育環境の不備、ビタミンA欠乏などによる免疫力の低下が発症の要因となり、二次的に肺炎が引き起こされます。
カメの肺炎の臨床症状
初期の肺炎では軽度の開口呼吸と変な呼吸音がみられます。肺炎が重度になると苦しそうに口を開けて大きく呼吸をすることを示すようになります。水生種であれば水に入りたがらなくなり、水の中では体の片方が沈むような傾くこともあります。肺炎の症状が現れず、体力や食欲の低下しかみられないこともあります。
カメの肺炎の診断
カメの肺炎を疑う場合にはX線検査を実施するのは一般的です。初期の肺炎はX線検査では明らかな異常が確認されないこともあるため、X線検査において異常がみられなくても肺炎の可能性は排除できません。画像検査からは感染因子の特定はできないため、気管洗浄液よりの細胞診や細菌培養検査を実施することも可能ですが侵入性が高い、鎮静・麻酔のリスクもあるのでほとんどの場合では実際的ではないです。
カメの肺炎の治療
病原体の特定することが難しいため、臨床現場では症状から感染因子を予想して、試験的に抗菌薬を使うことが多いです。注射や内服の投薬以外、場合によってネブライジングの効果も期待できます。
【Pneumonia in chelonians】
Occurrence
There is a wide range of etiologies associated pneumonia and infectious causes (bacterial/fungal/viral/parasitic) are most common. Risk factors include vitamin A deficiency, inappropriate husbandry, and any other stressors that can lead to immunosuppression in chelonians.
Clinical signs
Symptoms of early-stage pneumonia in chelonians include mild open-mouth breathing and wheezing nasal sound. In severe cases forced open-mouth breathing and dyspnea are observed. In turtles additional symptoms such as increased time spent on land and buoyancy disorders may also be noted. Sometimes nonspecific signs such as lethargy and anorexia may be the only clinical signs observed.
Diagnosis
Radiography is a very useful tool in diagnosing pneumonia in chelonians. Nevertheless, even if radiographic findings appears to be normal, the possibility of pneumonia cannot be ruled out. Fluid collected via tracheal lavage can be used for cytology and bacterial culture. However due to its aggressiveness and higher risks, it is not routinely performed in chelonians.
Treatment
Systemic antibiotics, preferably according to sensitivity panel for at least 4-6 weeks may be necessary in most cases. Depending on the species of the patient and severeness of the disease, nebulization can be of great help. Identifying and correction of underlying causes are also of great importance.
